MMHA Dispute Resolution Process
This form is to be used by MMHA members for the purpose of submitting a formal written complaint regarding non code of conduct issues. Upon completing this form, your complaint will automatically be submitted to our Risk Management Coordinator. Correspondence sent through other channels will not be addressed. A copy of your complaint will be emailed to the address provided. You will be contacted within 72 hours of submitting your complaint.

NOTE

All MMHA Code of Conduct complaints must be submitted through the Hockey Canada Safe Sport/Independent Third Party (ITP) Complaint process at https://sportcomplaints.ca/
